  • Anterior: 95%, bei Außenrotation/Abduktion
  • Posterior: v.a. nach Polytrauma/Krampfanfall
  • Begleitverletzungen: Rotatorenmanschettenruptur, Abriss Tub. majus, Bankard-Läsion / Hill-Sachs-Delle, Luxationsfraktur Humeruskopf (siehe Schultertrauma)

Workup

  • A: Habituell? Trauma?
  • U: Gelenkkopf palpabel? pDMS (N. axillaris)?
  • Röntgen: Schulter a.p. + Y-Aufnahme
    • Rim Sign: H.a. posteriore Luxation in a/p (Überlappung vorderer Pfannenrand mit Humeruskopf >6mm), oft übersehen

Management

  1. Reposition (Milch):
  2. Adduktion OA, Flexion Ellenbogen 90°
  3. Langsam Außenrotation 90° + Abduktion 90°
  4. Axialer Zug am OA, ggf. Druck Humeruskopf → Glenoid durch Assistenz
  5. Ruhigstellung: Gilchrist für 1 Woche
  6. Röntgenkontrolle
  7. Procedere: Ambulant UCh in 3d, MRT im Verlauf insb. bei jungen Pat. (schon 1. Luxation = Präarthrose)
