Rapid Reference

1. VEL (Cave Volumenstatus), Ziel-Diurese 200 ml/h, ggf. + Furosemid, Blasenkatheter
2. Rasburicase 0.2 mg/kg i.v.
3. HyperK+-Therapie
4. Dialyse großzügig

Cairo-Bishop-Kriterien

• Harnsäure >8 mg/dl oder +25%
• Kalium >6.0 mmol/l oder +25%
• Phosphat >1.45 mmol/l oder +25%
• Calcium <1.75 mmol/l oder –25% (in BGA)

Anamnese

• S: Dyspnoe? Ü+E? Diarrhoe? Fatigue? Urin? Krämpfe? Krampfanfall? TLOC?
• M: Prophylaxe (Allopurinol, Febuxostat)? Nephrotox?
• P: Schnellproliferierende Tumoren (z.B. SCLC, Keimzelltumoren, Leukämie, Burkitt-Lymphom)? Therapie mit bispezifischen AK oder CAR-T?
• L: CTX? (typisch TLS 3-7d nach Ende)

Workup

• EKG
• Labor: BGA, Profil “Bauchschmerz”, Ca2+, Phosphat, HST, Harnsäure, LDH, Albumin
• Urin: Status, Krea, Harnsäure
• POCUS: Harnwege (postrenal AKI?)
• Blasenkatheter (Bilanzierung)
